Bug: Avatar Icons have wrong tooltip in approval

1. When I added an additional approval, the tooltip on my avatar is duplicated from DZ's

Original description

Couple of issues here:

1. Sean and I are listed as additional approvers, but the text next to our avatars just says "Approved", which makes it look like we both approved it.
2. The existing approval (from DZ) has a tooltip on the avatar; the additional approvers do not
3. When I added an additional approval, the tooltip on my avatar is duplicated from DZ's

Approved. Approved. Aaaand it's lost all meaning.
