MR Template: Fix assign quick action, copy internal handbook template with comments

Why is this change being made?

While creating !163 (merged) I noticed that the MR is assigned to @me with the quick actions at the bottom. The label quick action does not seem to work too.

This MR fixes the assignment action, and also copies the MR template from the internal handbook which includes HTML comments that are helpful for team members to follow the same process as in the current handbook.
