Added two additional options and fixed a trivial case

Two new options added: (1) to allow empty lines in within comment blocks to include comment styling, and (2) to allow start/end lines to be maintained with comment styling.

The trivial case fixed is that of a completely empty comment, which is simply ignored. /**/
