Improve the incident template a bit for better clarity

What does this MR do and why?

I propose two improvements to the incident templates:

Pipeline summary as a table

Better formatting of the attribution sentence

Expected impact & dry-runs

These are strongly recommended to assist reviewers and reduce the time to merge your change.
