Simplify feature flag rollout issue
What does this MR do and why?
- This was previously simplified via !132533 (merged)
- I was just creating a feature flag and was reminded that the comments at the top actually were more confusing for me than helpful. And the placeholder references at the end I had never even noticed before.

Reasoning
I am removing many of these fields because, if you look at a random selection of feature flag rollout issues, they are rarely filled out. And when they are, the information is so vague it isn't really that helpful.
The noisier this template is, the more likely it is that I won't fill out any of it at all. By keeping only the highest value information, we are making it more likely that people will fill it out.
