Problem to solve
Access Requests using issues needs a streamlined mechanism to make a request. We want to be able to tie our tech stack to an access request. A user should be able to easily choose only the systems they want access to and the type of access they need.
This could be some sort of form with dropdown fields or ???

Anyone in GitLab needing access to a system, vault, group, channel

This is a pain point for IT Ops and new GitLabbers learning GitLab for the first time. It's creating a lot of backlog for IT because they need to ask for people to finish filling out the table, reduce the amount of information they have left in the table in order to understand what isn't being requested, fix or add back in instructions someone accidentally deleted, etc.

I'd also request this feature in order to prevent people from changing templates-if someone has a baseline role based entitlement, they should only be able to open the template, fill in their personal info and not change the baseline role entitlements.

- Anyone being able to quickly open an access request without needing help. Especially new people and people who aren't developers.
