refactor: FeatureStateManager to LSFeatureStateProvider

Description

Change all instances of FeatureStateManager to LanguageServerFeatureStateProvider to more accurately reflect the function of the class.

This is also inline with other providers that are being created for the Diagnostics page.

How has this been tested?

Run the extension locally and smoke test that nothing that changed in functionality.

  • If src/browser or src/common has been modified, please consider interoperability with the Web IDE. See Running the Extension in WebIDE.
  • Consider an end-to-end test for significant new features that aren't covered by integration tests.
