feat(Modal): Clarify guidelines and structure

Purpose

In a Slack thread (internal) it was mentioned that the modal guidelines could be more clear as it relates to the heading. This aims to add clarification while updating the behavior and accessibility notes.

Note that this update infers that a modal title is required and so any UI kit or GitLab UI examples should be updated.
