Update UX Transition template to be used by anyone in the UX dept

The language used in the UX Transition template focuses on product designers, but this template can essentially be used by anyone in the UX Department that is transitioning into a new team, or needs to handover information about a specific project or product category.

I've made some updates to replace mentions such as designer with DRI.
