In !31206 (merged) I updated the title for the members of the Monitor Stage to match the new convention from !31100 (merged)

However, the team lists depend on string matches so I am now updating the handbook pages for Monitor:APM and Monitor:Health. Without this the team member lists will be blank.

@sengelhard can you review this one as well?
