Say where a team member gets a licence and a cloud project

The docs never said how a team member gets a license or a cloud project. This adds that.

  • docs/getting-started.md: a "Before your first VM" list. Each item names what to get, links its source and gives the lead time. The offline file and the Self-Hosted Duo Agent Platform add-on need a Fulfillment request.
  • gce/bases.md: the license facts merged into the License section, with the staging mode they need.
  • README.md: one line in Install pointing at the new list.
  • CHANGELOG.md: one entry.
