Follow the version-specific changes type on upgrade pages

The docs style review now covers entries on version upgrade pages. Such an entry follows the version-specific changes topic type, copies the form of a similar entry on the other version pages, and gives the fixed patch level in Affected versions once a fix ships.

A reviewer raised this on an upgrade note whose entry combined an intended change with a bug a later minor release fixed.
