Add bug intake process

Why is this change being made?

The handbook has existing triage guidance, but it's not well understood how our entire process fits together to result in fixing bugs on time, and communicating our decisions to the users.

After working on bugs for a while I feel our process could be updated to improve our handling of bugs.

The high level goals are better Transparency, Bias for action, and Result for the customer.

What

• show how Triage, Refinement, Planning, and Reprioritization fit together
• make clear how is DRI for which aspect
• add reprioritization phase
