Add Red Team "Request for Disclosure" Process

Why is this change being made?

This MR adds a new process where any manager at GitLab can submit a "Red Team Disclosure Request". This provides an official channel for all managers to submit evidence explaining why they believe something they observed is a Red Team operation.

This MR is supported with another that adds an issue template for managers to use.

We are doing this to make stealth operations go more smoothly - providing an additional communication channel for those who are not "trusted participants" of a Red Team operation but might still need to request an operation be disclosed.
