docs(README): update with new location & installation

Fixes #1 (closed).

Rewrites the documentation to match with the current Antora 3.0+ version. Some throwbacks to the older version, but for the most part only existing behaviour is documented or included.

Thus far the installation instructions are invalid as the package does not exist.
