sv_maxidle* refinements

bones_was_here requested to merge bones_was_here/sv_maxidle_playertospectator into master

@terencehill I changed the description of sv_maxidle_spectatorsareidle as it's not assuming specs are always idle, but just also running idle timeouts for them. Should the cvar be renamed too?

Maybe #2410 (closed) would be best addressed by enabling sv_maxidle_playertospectator by default? I'm using 60 on my server.
