Disable CTS recording damage taken(/dealt) to scores

Forbid CTS from adding player damage dealt or taken to scorefields as its meaningless and those scoreboardfields are not even shown by default.

I do not see any value in having a running total damage taken which doesn't reset on respawns and that would be meaningless for xonstats too as it would only record damage taken in the interrupted run when the match ends.

Currently only suicides are not recording damage taken.

I'd also wish for removing damage dealt and damage taken scoreboard columns from "all" columns as they're meaningless but I was unable to figure it out myself.
