[FIX] Multiple fixes for captcha handling

SoftStart Code requested to merge softstart/tikiwiki/tiki:fix-missing-captcha into master

Fix warning with reCaptcha when the cache key is not set in session. Assure there is only token or question being generated by page load, even if antibot.tpl is included multiple times. Include captcha also in the remind my password page.
