[FIX][UI] bs5: A few small interface adjustments

Made the page lock icon less prominent; added Bootstrap toggle switch classes and adjusted form width so filter icon doesn't wrap in some fonts; reduced the large size of the link icon on headings; removed width of module shade icon so it isn't overlapped by module title, etc.
