📝 add gitlab issues template, code of conduct and RFC process

The MR introduce guidelines for contribution to switcher, through the use of gitlab issue system.

Issues templates will be active only once in the default gitlab branch (here the master branch).

Once propagated to the default branch, issues from the current issue system (a private JIRA) will be manually migrated into gitlab, and eventually closed.
