VFIO 9.2 backports

Testing: Intel: GVT-g, NVIDIA GPU, PF/VF NIC assignment, hotplug. AMD: PF NIC assignment, hotplug.

Backport of vfio and mdev changes and collateral in i915 and iommu to v6.0.

QE has noted several RHEL regressions relative to GVT-g behavior, these regressions also exist in and match the behavior of upstream v6.0 and are therefore tracked in a separate bug, bz2132752, where we'll work with upstream to try to follow-up with fixes for the most serious of these issues. GVT-g is not officially supported on RHEL and upstream maintenance has declined due to lack of support on latest Intel processor generations.
