Qblox backend - Fix missing signal on O2 and O4 outputs of baseband modules

Explanation of changes

Make channel map parameters connect_out_i = Q when io_name = real_output_i for i={1,3}.

Motivation of changes

No signal is produced in outputs O2 and O4 of baseband modules when io_name is real_output_1 or real_output_3.
