initramfs UUID and performance fixes

Caleb Connolly requested to merge initramfs-fixes into master

It turns out blkid wasn't pulled in explicitly, add the missing dependency and fix the last few uses. Additionally, adjust the pmos_boot(_uuid)/pmos_root(_uuid) handling to behave consistently and not be dependent on the order of the options in kernel cmdline.
