Add default issue template

Luca Weiss requested to merge issue-template into master

Previously this was set in the project settings, but according to the GitLab documentation this can now be set using this file since GitLab version 14.8. Keep it here to make sure it's version controlled as well.

@ whoever merges this, "This doesn’t overwrite the default template if one has been set in the project settings." so the template in the project settings needs to be deleted
