Add support for positive diagnoses.

As we're going to be treating people with a positive diagnosis (as ascertained by a positive PCR test right now, but there might be other reliable ways to diagnose this in the future) somewhat like vaccinated between 27 and 180 days, and fully vaccinated if they get a shot afterwards, we're creating a special procedure type for it.
