Sign up refactor

Brendan can you review the UX. @nliles can you look and comment as well? I didn't do a full effects driven form and kept some of the original hasStarted variables in the container. It's such a small form I don't think it matters. Also Brendan may want to simply redo the dumb component or just part of sign up.

Brendan - One concern I had was logging out if you don't want to continue the registration process. You can hit the menu and log out - but not sure the user would expect to be instantly logged in. I'm sure there are other minor issues but it seems to generally work fine.
