docs: Update model documentation guidelines and create-model.py script

This updates the model documentation guidelines and the create-model.py script portion regarding the documentation.

This is an effort to establish clear guidelines to create more navigable and uniform documentation across all models. This follows what was discussed in previous online meetings and in issue #1139.
