WIP Add images and stylesheet

@alf-eaton The further I go with this the more it seems like we are marrying the html converter to its consumer.

I think perhaps the right approach is to return the <article> element and everything below it and leave it up to the server to add the head and frontmatter.

Don't merge - let's discuss further.
