Fix `flattenEntrypoints` to consider root annotation

Description

Problem: currently this thing does not account for root annotation and that's pretty weird (every method working with ParamNotes should consider it entirely with 95% probability).

Solution: update flattenEntrypoints and, as was required, mkEntrypointsMap to work with ParameterType.
