Support for primo and secondo page layout

`Classical' layout of four-handed piano music positions the staves for the
left player (`secondo') on the left (even) pages, and the staves for the
right player (`primo') on the right (odd) pages. An additional constraint
is that the number of bars on the opposing pages must be equal. Ideally,
the page breaking algorithm is also capable to make either the left OR the
right player turn the page while having a rest.
Her an example for the layout:
page secondo primo
---- ------- -----
2 1-40 --
3 -- 1-40
4 41-70 --
5 -- 41-70
...
