Cleanup

ngc92/eigen:cleanup into master

some more cleanup, removing EIGEN_HAS_CONSTEXPR, EIGEN_HAS_INDEX_LIST, EIGEN_HAS_STD_RESULT_OF and Eigens own implementation of index/integer_sequence. These are the macros that were currently on the list here #2372 (closed), though there are some more where I'm not sure yet whether they should be removed.
