Update Nexus 5 Nougat caf kernel and change directory name from 'hammerheadcaf' to 'hammerheadcafcm'

The kernel is updated with the latest commits from CyanogenMod. The directory should be hammerheadcafcm, not hammerheadcaf, or it doesn't build.
