Adjust health facilities icon scale on map

Instead of making the icon size vary linearly with the hospitalization rate, we separate it in 5 bins with a determinate size for each bin. The values were chosen by 1) ploting a histogram of the hospitalization rates to have an idea of the distribution, 2) selecting the threshold values, and 3) visually testing sizes to get a better visualization.
