[agent] plage ouverture fixes : fix monthly recurrences + dedicated form pages

2 fixes :

  • wrong date format in moment with new datepicker was causing misinterpretation of the dates => start date were messy with monthly recurrences
  • navigating in datepicker was erroneously navigating away from form sidebar for some browsers in some situations. simple and rough solution : move away from sidebar forms to dedicated pages :)
