new output templates

Paulette Lieby requested to merge dev into master

a few new templates, all generic:

  • one that shows the km all expanded, with exactly the same design as the DSW Questionnaire Report 1.3.0
  • two further ones that show a slightly simplified output wrt DSW Questionnaire Report 1.3.0

All these templates are really only light tweakings from the original DSW Questionnaire Report 1.3.0.
