hpcrun: remove level0-kernel-module-map.{c,h}

hpcrun: remove level0-kernel-module-map.{c,h}

with qmcpack, there were deletions of kernels not inserted in this map. analysis of how to fix the issue revealed that no one looked up kernels in the map. the issues present were fixed by dead code elimination.
