Add rivet-mkhtml --no-rivet-refs option
Make make-plots work with multiple ratio plots and legends again, by getting rid of the separate treatment of the 1st and 2nd-9th instances. This propagates a few bug fixes, which were previously only applied in the 1st instance, also to the 2nd-9th instances.
