Fixed duplicate numerical entries in reference list.

Ref. 182 was used twice. Since 182 and 183 were cited together they were moved to the end of the list to 192 and 193. Ref. 185 was also used in two separate entries. One of them was changed to 183.

Missing raw html directives, in many of the recent entries, were also added.
