Make output from grep/ack/ag clickable (with line numbers)

Tools like grep output one match per line, in a format similar to this:
<PathToFile>:<LineNumberOfMatch>:<SomeContextAroundMatch>
for example: ./Foo.txt:37:Foo Bar Baz
It would be awesome if iTerm could parse this output and make each line clickable (like URLs). When you click on the line, it should invoke the default editor and make it jump directly to that line. This is supported by many editors:
Vim: $ vim +<LineNumber> <FileName>
Sublime Text: $ subl <FileName>:<LineNumber>
