Weird Escape Sequences (Occasionally) in "vi"

Occasionally, vi (vim) will go wonky on me -- across all tabs, in all windows, and regardless of whether I'm SSH-ed into another box or running an editor on my local machine...
Just now, any time I'd hit the number "6", vi would receive an "ESC-O-v" sequence (at least that's what appeared if I hit ctrl-v first...) Another time, hitting "/" -- whether in insert mode or in command mode trying to search -- would generate another weird escape sequence.
And like I say, this will happen regardless of what machine I'm logged into, etc. But if I open up Terminal.app, things will work a-okay there. And outside of vi, bash has no problem.
I don't have any weird ".vimrc" options, or anything (Apart from "noautoindent" and "modelines=4", but the fact that it happens on machines where I don't even HAVE a vimrc is peculiar.) I've tried turning off and on "Disable save/restore alternate screen" and other settings, but nothing seems to jump out as a Smoking Gun.
Previously, it seemed like the only way to fix the problem was to Quit and restart iTerm, but now, for whatever reason, the problem seems to have gone away all by itself. (RARGH!!)

Build 1.0.0.20130622 on Mac OS X 10.8.4, but it happened before this, too.

I know I haven't really provided a lot to go on, but I'm hoping something will click with somebody ("Escape, Capital-O, Vee... AHA!!") 'cuz otherwise, I'm stymied...
