fix: update report version from 14.0.0 to 14.0.3

What does this MR do?

With !10 (merged) we added the latest fields matching schema v14.0.3 but forgot to bump the report version itself.

I'd love to find a programmatic solution here but none immediately come to mind. Instead I updated the default MR template to include a checkbox item for ensuring the reports match the schema version
