Add support for on-premise GitLab

Add --host-url option in .gitlab-ci.yml example, it could save minutes or hours for people who want to try triage on a self-hosted GitLab.

Yes, I'm the one who just tried to setup and deploy triage on our company's GitLab, and spend lots of time in fighting with "auth failure" error... 😅
