Document the two processes for deployment failures

As part of gitlab-com/gl-infra&752 (closed) Release Managers will move to only opening incidents when EOC or dev-escalation is needed.

  1. This MR updates our Deployment failures doc in preparation for this change.
  2. Updates the process for handling QA failures. This change follows several months of experimenting with using release issues instead of incidents for certain failures - https://gitlab.com/gitlab-com/gl-infra/delivery/-/issues/1221#note_957660892
