Update weekly deployment blocker report template

What does this MR do and why?

One of the steps in the weekly deployment blockers issue is to copy and paste the table of incidents to the yearly epic. This creates duplicate information that is difficult to maintain if any of it needs to be changed. In this MR, we change the process to not copy the table but link back to the weekly issue instead.

The "Weekly Overview" section of the epic will be removed after this is merged.
