Update README with up-to-date data (August 2025)

Context

I worked a lot on triage-ops recently (the reactive part), and I found that the READMEs needed to be updated.

Now that I have a lot of context on how it works, it feels like the best time to do this update.

What's in this MR

  1. Updated triage-ops README to have more useful information
  2. Refactored the two existing READMEs for scheduled and reactive parts of triage-ops
