Add Omnibus support for Puma

Closes #3846 (closed)

This builds atop of !2783 (merged).

To do:

• Add more specs
• Add documentation
• Add to gitlab.rb.template
• Fix Puma worker killer limits to account for total RAM usage
• Fix any dependencies that check for Unicorn rather an a application server
