Fix DeclarativePolicy condition scopes

What does this MR do and why?

Describe in detail what your merge request does and why.

This MR adjusts the scopes of some DeclarativePolicy-derived classes. Some were missing, leading to cache misses, others were incorrect. I made the changes based on the documentation at https://docs.gitlab.com/ee/development/policies.html#scope.
