Update and simplify the `create blank project` form

• removes description from blank project form
• tracks user input in the path/slug field so we can quantify usage
  • tracking proof response.json

How to set up and validate locally

1. Go to create a new project
2. Click 'Create a blank project'
3. Observe the description is no longer present.
