What does this MR do

I have slightly revised the documentation for the npm gitlab package registry.

A Table of Contents was added as well as slight revisions to the layout in the publish section.

Why?

This document is very lengthy and quite confusing as is. When setting up a gitlab package registry I had to use many different resources just to get it working. Simply ordering everything as if it's a step by step guide when it is not in fact not exactly a step by step guide was not wise and the table of contents helps users navigate with more ease.
