Remove file_type uniqueness for PipelineArtifact

What does this MR do?

This MR removes a uniqueness constraint at the application level for file_type in our PipelineArtifact model.

Why are we doing this?

Since a pipeline artifact is inserted by the system, we'll never surface it to users. We can simply remove it as we don't need it.
